Which forms you get
Tattoo and piercing forms are included in the base package. Additional forms such as tooth gems can be added optionally. The process always stays the same: clean, traceable, secure.
Tattoo form
Consent + health details + signatures — structured and complete.
- Client data, contact details, date of birth
- Health questions and relevant risks
- Consents and studio texts (dynamically from settings)
- Event mode compatible (conventions/walk-ins)
- Motif, stencil and reference images storable per consent
- Signature(s) directly in the form
- Touch-ups with separate signature and date (cleanly separated legally)
Piercing form
Piercing-specific questions incl. special cases — without missing anything.
- Piercing location, previous piercings, complications
- Relevant medical details (e.g. blood pressure, allergies etc.)
- Event mode compatible (conventions/walk-ins)
- Option to decline with documented reason
Tooth gem form
Tooth gem-specific questions with clear risk disclosure and clean documentation.
- Dental status (e.g. cavities, gum problems, dentures)
- Relevant medical details and allergies (e.g. adhesive, latex, nickel)
- Disclosure about risks such as enamel damage or detachment
- Consent and signature specific to tooth gems
Cosmetics form
Cosmetic and treatment forms with type-dependent disclosure — compliant, traceable and cleanly documented.
- Selection of cosmetic type (e.g. PMU, microblading, lashes, further treatments)
- Individual disclosure and aftercare texts per type (each treatment with its own risk and info texts from studio settings)
- Relevant health questions (e.g. skin conditions, eye conditions, allergies)
- Treatment-specific special cases and exclusion reasons
- Consent and signature matching the selected treatment
- PDF documentation with exactly the text belonging to the treatment type
Protection in the workflow
The strength isn't 'digital'. The strength is: TattooMate doesn't let you build dangerous gaps.
Validation & required fields
If important details are missing, it doesn't just 'save anyway' — it guides cleanly.
- Required fields: name, date of birth, consents etc.
- Conditional fields: details only when needed (e.g. for allergies)
- Clean structure instead of endless freetext
- Fewer errors under pressure (walk-ins, events)
U18 workflow with block
Under age means nothing gets 'waved through'. The system enforces the correct process.
- Service-dependent minimum age (e.g. piercing from 12, tattoo from 18). Age limit is set per form and determines what is permitted at all.
- Parent/guardian data mandatory
- Signatures + uploads mandatory
- Submission blocked until required sections are complete
