Tattoo and piercing forms are included in the base package. Additional forms like tooth jewelry can be added optionally. The workflow stays the same: clean, traceable, safe.
Tattoo form
Consent + health information + signatures — structured and complete.
- Customer details, contact details, date of birth
- Health questions and relevant risks
- Consents and studio texts (dynamic from settings)
- Event-mode ready (conventions/walk-ins)
- Motif, stencil and reference images can be saved per consent
- Signature(s) directly in the form
- Touch-ups with separate signature and date (clean legal separation)
Piercing form
Piercing-specific questions incl. edge cases — so you don’t forget anything.
- Piercing location, previous piercings, complications
- Relevant medical information (e.g., blood pressure, allergies, etc.)
- Event-mode ready (conventions/walk-ins)
- Option to reject with documented reason
Tooth jewelry form
Tooth-jewelry-specific questions with clear risk disclosure and clean documentation.
- Dental status (e.g., cavities, gum issues, dental prosthetics)
- Relevant medical information and allergies (e.g., adhesive, latex, nickel)
- Disclosure of risks such as enamel damage or detachment
- Consent and signature specifically for tooth jewelry
Cosmetics form
Cosmetics and treatment forms with type-specific disclosure — legally sound, traceable and cleanly documented.
- Select the cosmetic type (e.g., PMU, microblading, lashes, other treatments)
- Individual disclosure and aftercare texts per type (each treatment with its own risk/info texts from studio settings)
- Relevant health questions (e.g., skin conditions, eye conditions, allergies)
- Treatment-specific special cases and exclusion reasons
- Consent declaration and signature matching the selected treatment
- PDF documentation with exactly the text belonging to the treatment type

Validation & required fields
If important information is missing, it won’t “save anyway” — it guides you properly.
- Required fields: name, date of birth, consents, etc.

Under-18 workflow with lock
If underage, nothing gets waved through. The system enforces the correct process.
- Service-specific minimum age (e.g., piercing from 12, tattoo from 18). The age limit is set per form and determines what’s allowed at all.
- Parent/guardian details mandatory
- Signatures + uploads mandatory
- Submission blocked until required parts are complete
